Hypochlorous acid is one chlorine and one hydrogen bonded to an oxygen. It is small, it carries no charge, and that is the whole reason it works the way it does.
In water HOCl sits in balance with the hypochlorite ion, and below roughly pH 6 the active HOCl form dominates. That is why the formula is pH balanced rather than simply mixed, and why it is held in isotonic saline: the nose is asked to accept something close to what it already contains.
